Creating clear pathways to better care.

We help providers become easier to find, easier to trust, and easier to choose.

We help people and organizations move through complex care decisions with clarity, confidence, and relationships that last. We start where the need is sharpest, so families and referrers can decide with a clear standard instead of guesswork.

No commissions. No hidden incentives. A flat fee, and the truth.

Quality first. Relationships second. Placement last. In that order, on purpose.
The problem we exist to fix

The system built to help families is quietly working against them.

Good homes sit empty.

Thousands of small, capable care homes have open rooms because families and hospitals can't find them, or can't yet tell whether they're a good fit.

Families decide in fear.

The people paid to help are usually paid by the very homes they recommend, a model now under a U.S. Senate inquiry and growing state disclosure laws.

No one is on the family's side.

There's no neutral party helping a good home become worth choosing, and no one who stays with a family when the first option is full.

What we believe, in one idea

When people understand their options, they make better decisions. When they can see clearly, they move with confidence. When organizations build trusted relationships, better systems follow. And better decisions build stronger people, stronger organizations, and stronger communities.

Everything we do comes back to that. To a family, it sounds like this: every person deserves clear guidance when life's biggest decisions become overwhelming. To an organization, it sounds like this: every organization deserves a steady partner when its most important decisions affect people's lives. Same belief. One firm, working from the boardroom to the living room and translating between them.

Our approach

We put things in an unusual order, and it's the whole point.

Most of the industry sells placement first and worries about quality later. We reversed it, because that order is the only way our promise of clarity and confidence can be true rather than a slogan.

1

Quality comes first

We start by helping a home become genuinely ready: safer, clearer, easier to understand, and prepared to serve families well. If the care isn't there yet, we help build it before anything else.

2

Relationships come second

Once a home is ready, we help it get found and chosen by families, by hospitals, and by the case managers who need somewhere dependable to send people. Confidence is earned steadily, and we build it deliberately.

3

Placement comes last

Connecting a family to the right home is the result of the first two steps done well. It's not the front door, and not something we'll rush or hide the incentives behind. We'll lose a sale before we spend your confidence.

Where most partners start with us

A readiness review: a clear, honest picture and a real path forward.

We look at your home against a consistent standard, then hand you a short, plain-language report and a 30-day plan. No jargon, no pressure, just what's working, what to fix, and what to do first.

  • How families and referrers find you, and why they may not yet.
  • How ready your home is to earn and keep the confidence of families and referrers.
  • A short, prioritized plan: what to change, in what order.
  • A flat fee agreed up front. No commission on who moves in.

What we do

Services that build on one another, offered one step at a time.

We add the next thing only when the last one is working. Nothing here depends on volume, and nothing pushes you toward a decision you're not ready for. Every engagement is priced plainly once we understand your situation.

Why it matters

A good home is a real home, not an institution.

Many of the homes we help are small and family-run, often owned by first- and second-generation immigrant families who give warm, capable care. Helping those homes get found and chosen isn't a side project. It's the work.

  • We coach before we judge. Every provider deserves a fair chance to improve.
  • We say what's true, including what's hard, even when it costs us a sale.
  • We keep the dignity of the person at the center of every decision.

Who we help

Three groups, one clear path between them.

We work for the people making the decision, not the ones paying the biggest referral fee. We understand the entire care ecosystem because we've worked across it.

Care providers

Small and family-run homes with open rooms and good care to give. We help you get found, get chosen, and get ready, for a fee you agree to up front.

Families

People carrying a hard decision, often at their most overwhelmed. We give you a clear way to compare, honest answers, and time to decide at your own pace.

Hospitals & case managers

People discharging patients into uncertainty, with no time to vet every option. We give you homes you can rely on, backed by a standard you can trust.

The Uzoria Path

A clear, steady way forward.

The same five steps guide every engagement, whether we begin with one care home or a health system.

1

Understand

We listen first, learning your situation, your goals, and where the real gaps are.

2

Prepare

We build readiness, so the care, the systems, and the story are clear and dependable.

3

Connect

We help the right people find one another, with the confidence that comes from clarity.

4

Strengthen

We reinforce what works and improve what doesn't, guided by evidence and honest feedback.

5

Sustain

We keep the pathway steady over time, so good care stays visible and easy to choose.

What we believe

Clarity is how the work gets done. Confidence is what you're left with.

01
People decide at their own pace.

We inform; we don't pressure. A care decision is not a transaction to close.

02
Clarity over jargon.

If a worried family can't understand it, we rewrite it. Plain words, always.

03
Every provider deserves a fair chance.

We coach before we judge, and we help homes improve rather than shut them out.

04
Dignity for the person at the center.

The decision is about a human being, not a placement or a room to fill.

05
Say what's true, including what's hard.

Transparency even when it costs us a sale. That's the whole basis of trust.

06
We grow only in ways that make care better.

Growth that erodes trust isn't growth. We'd rather stay small and honest.

Our story

We saw a gap that experience alone couldn't solve.

Across healthcare, public health, managed care, and state government, we kept seeing the same problem.

  • Families struggled to understand their options.
  • Providers delivered strong care but were difficult to find.
  • Hospitals worked hard to connect people to the right resources but lacked consistent pathways.

The challenge wasn't simply access. It was clarity.

That realization became Uzoria Partners. We help providers become easier to trust, easier to find, and easier to choose, so families can make confident decisions and organizations can build stronger referral networks.

Founded on experience. Built for partnership.

Uzoria Partners was founded by Dr. Linda Baggio, DrPH, MPH, whose career has spanned public health, Medicaid, managed care, health equity, provider partnerships, and state government. Our experience gives us perspective. Our work is measured by the results we create for partners.

Because better care begins long before a service is delivered. It begins with people understanding where to turn, who to trust, and how to move forward.
